Scheduling an Appointment
Appointments and Registration
Appointments for all our office locations can be made by calling our main telephone number, 317.926.1056 or 800.283.1056. The scheduling staff will work with you to find an appointment time and office location that will meet your needs. If you feel your medical condition requires special or immediate attention, please let us know so we can get a member of our staff involved in scheduling an appointment for you as soon as possible. Our phones are answered during normal office hours which are 8:00 a.m. to 5:00 p.m. Monday through Friday. After hours, calls are directed to our answering service.
Your First Visit
Please arrive 20-30 minutes early for your first visit to our practice.
As part of your visit, please bring the following:

Medical insurance card(s)

Drivers license or other photo identification

List of current medications (prescribed and over the counter)

List of medications

List of known allergies
-
Recent test results, including CT scan films, applicable to the reason for your visit

Referral form, if required by your insurance
If you have a co-pay with your insurance, we will collect it prior to you seeing the physician or advanced provider.
Office Schedule
We do our best to adhere to the appointment schedule. We appreciate the value of your time and share your dislike for long waiting times. Unfortunately, we sometimes run behind schedule. If this happens when you're scheduled to be seen, we will offer you the option to wait, return for a later appointment that same day if one is available, or reschedule your appointment for another day.
Insurance Information
CENTA participates in and has agreements with numerous health insurance companies. While we can normally tell you if the physician you are scheduling an appointment with participates in your particular plan, we encourage you to also contact your insurance company to verify whether the physician is a participating / in-network provider. Because we participate in many different plans, it is difficult for us to know exactly what services are covered or not covered under your particular plan. If you have any questions whether a service will be covered, again we encourage you to contact your insurance company before the service is provided. Your insurance company may require a pre-certification, prior authorization, or referral for some services, such as radiology tests and surgery. Receiving a pre-certification or prior authorization does not guarantee that your insurance company will pay for the service.
As a courtesy, we will file a claim with your insurance company after services are provided unless you notify us otherwise. To file your claim, it is important that you provide us with current, complete, and accurate information.
Normally, we will receive an Explanation of Benefits (EOB) statement from your insurance company within 30 to 45 days of filing your claim. The EOB will indicate the amount you are responsible for, based on your policy benefits. You may be responsible for a co-payment, deductible, co-insurance, or for the entire service if it's determined by your insurance company as a non-covered service. We will mail you a statement for the balance owed and would appreciate payment in full shortly after you receive it. If amounts due for services you've received become delinquent, your account may be referred to a collection agency and you may be dismissed from the practice. If you are unable to meet your financial obligation with us, please contact our Billing Department before your account becomes delinquent to discuss possible payment options.
Please remember that our relationship is with you to provide quality healthcare to you and/or your dependent. As a result, all charges for services received are your responsibility including payment for those services. We cannot always depend on your insurance company to make timely payment on your behalf.
